Beauty Bank is NeuroSpa’s Exclusive Loyalty Program. As a club member, you will receive the best rates on all services, products and member-exclusive perks. On the day you register you will have immediate access to club member rates. Your first monthly payment will begin after your sign-up date. Monthly payments will be available to use towards your services. All services that are rendered will need to be paid in full the day they are received.
NOTICE TO MEMBER: DO NOT SIGN THIS AGREEMENT UNLESS YOU HAVE READ IT IN ITS ENTIRETY.
Registration means you have read this agreement in its entirety, and you have no questions about membership.
BEAUTY BANK AGREEMENT & INFORMATION:
JOINING BEAUTY BANK: Both new and existing patients are eligible to join Beauty Bank. Upon completing the registration form, we will assist you with putting a card on file and establishing your billing cycle. Your monthly billing date will be the same as your sign-up date each month. For example, if you join on the 10th, your future payments will be due on the 10th of each month. Your first payment is due immediately upon signing up.
PROMOTIONAL RATES, OFFERS & PRIVILEGES: As a club member of Beauty Bank, you will receive club members-only rates, exclusive offers, and any additional membership privileges are guaranteed so long as you remain in good standing with your monthly payments and paying any remaining balance at your visit. These rates and offers are for the registered member only. Member’s only rates and offers cannot be given, purchased, or transferred to another person who is not a member of Beauty Bank. Beauty Bank rates and offers cannot be combined with any additional promotions or offers for additional discounts or savings. Rates, offers, privileges, and perks are subject to change at any time without notice. Beauty Bank rates and discounts cannot be applied to any packages or promotions.
MONTHLY BILLING / REGISTRATION / PAYMENTS: An active card on file is required for all Beauty Bank Memberships.
IN-OFFICE & ONLINE SIGN UP: Upon signing up for the Beauty Bank Membership at your in-office visit, you will immediately gain access to your new membership rates and perks. A $250 charge, which is considered your first monthly payment, will be due on the day you sign up and is applied directly to your Beauty Bank balance and available to use immediately.
MEMBERSHIP BILLING REQUIREMENT & PAYMENT AUTHORIZATION: Your account will be billed on the same day of each month as your sign-up date for $250 per month. This recurring payment will be automatically withdrawn from the credit/debit card on file on your designated billing date. Successful recurring membership payments are required to maintain active Beauty Bank status. Your funds never expire, but designated perks and services are only available at a discount so long as you are an active Beauty Bank member. Failure to pay your membership dues on time may result in your membership being terminated.
ANNUAL MEMBERSHIP:
You may choose to be billed annually. The annual membership cost is $3000. There are no refunds for this membership. You will be billed on the same day of the year as you started your membership. All funds are available for you to use immediately.
By signing up for Beauty Bank, you agree that recurring payments of $250.00 will be automatically withdrawn from the credit/debit card on file every month on your assigned billing date. These recurring monthly payments will continue until you cancel your membership (see cancellation terms). All sales are final and there are NO REFUNDS, for any reason. You may not move, stop, or delay payments. By attempting to do so, your membership may result in a permanent cancellation and/or penalties. Signing up for Beauty Bank allows NeuroSpa authorization to charge your card monthly for recurring membership dues, and additionally for: any treatments, procedures, services, products, or on-account credits that are purchased or rendered. If your available funds accumulated in your Beauty Bank are not sufficient to cover the cost of the any treatments, procedures, services, products, or on-account credits that are being purchased or rendered during your visit, you will be required to pay the remaining balance the day they are performed/purchased, prior to leaving the office.
UNSUCCESSFUL PAYMENTS & PENALTIES: You, the Beauty Bank Registrant, are solely responsible for the status of your Beauty Bank Membership. If your card on file declines or is unsuccessful for any reason, it is your responsibility to update the card on file with NeuroSpa. If your card on file declines, you will not be permitted to schedule any appointments until you have updated your card and payment has processed. Future appointments are subject to cancellation if your card on file declines. If your card on file declines, we make multiple attempts to reach you to attempt to get your account in good standing before your next billing cycle. If payment is not made and/or card on file is not updated within 15 days, you will be subject to a $50.00 late fee. NeuroSpa will attempt collection of your Beauty Bank dues/fees after receiving information of a card decline. In accordance with applicable law, if the first attempt to collect any fee under this agreement is unsuccessful, we may make additional attempts to collect from any/all payment methods you provide us. We may attempt collection via phone call and/or text message to the phone number on file for up to 30 business days after initial decline. After the first initial attempt to collect payment, a $25 service fee may be applied for each instance we submit or re-submit such payment request and it is returned uncollectible for any reason including but not limited to: insufficient funds, expired or cancelled payments, overdrafts, or closed accounts. We are not liable for any fees charged by your financial institution in the event a payment request is returned uncollectible. Failure to pay your balance will result in back-paying for any payments missed and termination of account. Beauty Bank Members who do not update their card on file for membership and pay their balance due within 30 days of initial card decline will automatically be cancelled and immediately forfeit all perks and discounts.
CANCELLATION: HOW TO CANCEL: To cancel your membership, you must send a 30-day notice written request via email to info@theneurospa.com. Once your cancellation request is received, you will be sent a link to formally cancel your membership. You must complete this form to confirm your cancellation request. Failure to do so will result in delays in cancelling your membership. Once your membership is cancelled, you will no longer be able to receive Beauty Bank perks.
FUNDS ON ACCOUNT AFTER CANCELLATION / REFUND REQUESTS: If you have funds remaining on your account after membership has ended you will be available to put your credit toward treatments, procedures, services or products at regular price. Please note that there are NO REFUNDS, and all sales are final. There will be no expiration date for your funds.
If you choose to cancel, your funds will remain accessible to you as a credit at NeuroSpa. There will be no time limit imposed on using these funds. However, please note that once your membership has officially ended, you will no longer have access to members-only rates, offers, and privileges. There are NO REFUNDS, and all sales are final. You may not request a refund for any funds that you have in your Beauty Bank.
REINSTATEMENT: Memberships may be reinstated if the account was in good standing and approved by the cosmetic director. You may reinstate your membership at any time to receive your members-only rates, offers, and privileges. If you decide to reinstate your membership, you may be required to pay a registration and/or processing fee at sign up and your membership will begin as if it is new, on the reinstated date.
FREEZE: Some members of Beauty Bank may be presented with the option to "Freeze" their membership. To be eligible for Freeze, you must not have any prior declines. Freeze may not be offered to all patients and is not always available during the calendar year. Freeze has a separate agreement that will be sent to the Beauty Bank Registrant if they are eligible. When Freeze is a presented option, it is only available for up to 6 months per registrant and costs $25 per month. A beauty bank account that is frozen will be charged $25 per month which will be applied toward your beauty bank fund.
Additional Terms & Conditions:
A. PARTIES
• NeuroSpa and you, "Beauty Bank Registrant" agree that by signing this agreement, you signed up for a recurring monthly membership and agree to all the terms in this agreement. The terms "you/Beauty Bank Registrant" and "NeuroSpa" include heirs, estates, agents, representatives, officers, directors, shareholders, successors, affiliates, subsidiaries, and employees. Both parties make this agreement on behalf of, and it binds all these included persons and entities. It is your responsibility to notify NeuroSpa of any change in your address, phone number or name change, and billing changes.
B. REPRESENTATIONS
• Physical Condition & No Medical Advice: You represent that you are in good physical condition and have no medical reason or impairment that might prevent you from your from receiving any treatments, procedures, services, or using recommended products at NeuroSpa. As such, you acknowledge that NeuroSpa did not give you medical clearance relating to your ability to obtain said services. If you have any health or medical concerns now or after you join, discuss them with your physician prior to receiving services.
• Liability for Property: NeuroSpa is not liable to you or your guest(s) for any personal property that is damaged, lost, or stolen while on or around NeuroSpa premises including, but not limited to, a vehicle or its contents or any property left in or around NeuroSpa facilities. If you or your guest cause any damage to NeuroSpa’s facilities, you are liable to NeuroSpa for its cost of repair or replacement.
• Entire Agreement & Enforcement: You acknowledge that neither NeuroSpa, nor anyone else, made any representations or promises upon which you relied that are not stated in this agreement. This document contains the entire agreement between you and NeuroSpa and replaces any oral or other written agreement. If a court declares any part of this agreement invalid, it will not invalidate the remaining parts, which continue unaffected. If NeuroSpa does not enforce any right in this agreement for any reason, NeuroSpa does not waive its right to enforce it later.
C. MEMBERSHIP
• Your membership permits you to receive discounted pricing for specified treatments, procedures, services, and products at NeuroSpa. Your membership permits you specific members-only privileges. Your membership is subject to all current company policies, rules and limitation. Your membership gives you no rights in NeuroSpa, its management, property or operation. NeuroSpa may assign, transfer, or cancel your membership in its sole discretion. NeuroSpa can offer memberships at different rates and terms than yours.
D. RULES AND REGULATIONS
• You agree to follow NeuroSpa membership policies and spa rules. NeuroSpa may, in its sole discretion, modify the policies and any rule without notice at any time. Policies presented to patients via text message, intake form, sign up form, and verbal communication shall be considered a part of the rules of NeuroSpa. NeuroSpa reserves the right to terminate your membership immediately for violation of any membership policy or rule.
E. PRACTICE & MEMBERSHIP POLICIES & BOOKING AVAILABILITY
• NeuroSpa reserves the right at any time to delete, discontinue, modify, or replace any services without any effect on this agreement. NeuroSpa also reserves the right to make changes to quantity services offered and to alter the hours of operation in NeuroSpa’s discretion. You will be offered priority booking however you acknowledge that the services in the facilities are available subject to demand and are offered on a "first come, first serve basis". NeuroSpa does not
• NeuroSpa highly requires all patients book appointments to ensure they will be seen on a date and time that works best for their schedule. By joining this membership, you have read and understood the booking and practice policy at NeuroSpa.
F. DUES, FEES, CHARGES & TAXES
• You may use your available Beauty Bank balance at your appointments to cover either your full balance or partial balance, if applicable. If the balance for treatments, services, or products rendered at your appointment is greater than your available Beauty Bank balance, you must pay the remainder either prior or immediately after the service, before leaving the facility.
• Beauty Bank Membership requires an active card on file to process the recurring monthly payment to your bank, your remainder balance (if any) at your appointment, and any additional fees you may incur.
• Payment Authorization: You have full control over the payment authorization and can cancel your membership according to the guidelines listed above. You must notify NeuroSpa within 30 days of a claimed error on your bank statement or credit card statement. If you claim your monthly dues were not stopped when you told NeuroSpa, you must have written proof or NeuroSpa will not reimburse you for any deductions which you claim should not have been deducted.
• Deposit, Cancellation, and No-Show Policy: No deposit is required at booking for most appointments for active Beauty Bank members in good standing, after the initial consultation. With limited appointments available, we strive to accommodate to every client’s busy schedule. We sincerely ask that our clients provide us with at least 24 hours’ notice if you are unable to keep the time we have reserved for your appointment. Any cancellation or reschedule made less than 24 hours prior to your appointment will result in a cancellation fee of up to $500, depending on the appointment time and/or number of offenses. If you are more than 15 minutes late for your service, we may not be able to accommodate you. In this case, the same cancellation fee will apply. We will do our best to reschedule your service for another time that is convenient to you if we are unable to accommodate you on your original appointment date. Each time a patient misses an appointment without providing proper notice, another patient is prevented from receiving care. Therefore, NeuroSpa reserves the right to charge a fee of up to $500 for all missed appointments ("no shows"). Any fees for last-minute rescheduling, cancellations, or no-shows will be subtracted from your Beauty Bank balance if applicable. If your Beauty Bank balance does not have the funds require to pay the balance, your card on file will be charged an additional amount. Your monthly Beauty Bank balance will be charged at its usual time and additional fees will not impact your payment schedule.
G. COMMUNICATIONS
• By becoming a patient at our practice, you acknowledge and agree to receive essential communications, including emails or text messages, regarding your appointments and promotional materials. It is understood that these communications are necessary for the effective operation of our services. However, you maintain the right to unsubscribe from these communications at your discretion, without any impact on your ongoing patient status or access to our services.
• To unsubscribe from our email communications, you can opt-out by following the instructions provided in the emails themselves.
• To unsubscribe from our text messaging service, you can simply reply with "STOP" or any other keyword specified in the messages to opt-out. Alternatively, you may contact our office directly and request to be removed from the text messaging service.
• Please note that even if you choose to unsubscribe from these communications, you may not receive essential notifications related to your appointments and the provision of our services.
